Liberty Hospice Earns Strong CMS Star Ratings for Patient Care and Family Caregiver Experience
Liberty Hospice is proud to share that many of our locations have earned high star ratings from the Centers for Medicare & Medicaid Services (CMS), reflecting our ongoing commitment to delivering compassionate, high-quality end-of-life care to patients and their families.
CMS uses a five-star rating system to help patients and families compare hospice providers based on measurable quality outcomes and family caregiver experience. These ratings provide an easy way to understand how different providers perform compared to others across the country.
We are honored that several Liberty Hospice locations received four-star ratings, demonstrating our dedication to excellent clinical care and positive family experiences.
What CMS Hospice Star Ratings Measure
The Hospice Quality Reporting Program evaluates how well hospice providers perform across several important quality measures. These are calculated using Hospice Outcomes and Patient Evaluation (HOPE) data, Medicare claims data and the Consumer Assessment of Healthcare Providers and Systems (CAHPS) Hospice Survey. Key quality measures include:
- Comprehensive Assessment at Admission: Evaluating seven care processes, including pain screening, treatment preferences and addressing beliefs and values
- Hospice Visits in the Last Days of Life: Assessing the proportion of patients who received in-person visits from a registered nurse or medical social worker on at least two of their final three days
- The Hospice Care Index (HCI): A score based on ten different indicators that capture care processes throughout the hospice stay
In addition to clinical performance, CMS publishes a Family Caregiver Survey Rating based on feedback from the CAHPS Hospice Survey. This survey gathers feedback from family caregivers about their experiences, focusing on:
- Communication with the Hospice Team: How well the team kept the family informed
- Getting Timely Help: How often caregivers received help as soon as they needed it, including during evenings and weekends
- Treating Patients with Respect: Whether the hospice team treated their loved one with dignity and genuinely cared
- Emotional and Spiritual Support: The level of support provided to both the patient and family, including after the patient passed
- Training for Caregivers: How well the team educated family members to care for their loved one, including managing pain medicine and symptoms
- Care Preferences: How the team respected the patient’s wishes while caring for them
- Help for Pain and Symptoms: Tracking how effectively the hospice team addressed issues like trouble breathing and feelings of anxiety or sadness
Agencies with ratings above the national average are performing at a high level, making these results a significant achievement for our teams. CMS updates these ratings quarterly to ensure the data reflects current performance and patient outcomes.
