CMS uses a five-star rating system to help patients and families compare home health agencies based on measurable quality outcomes and patient experience. These ratings provide an easy way to understand how agencies perform compared to others across the country.

What CMS Home Health Star Ratings Measure

The Quality of Patient Care Star Rating evaluates how well home health agencies perform across several important clinical measures. These include both process-of-care measures and patient outcome measures, such as:

• How quickly care is initiated after a patient is referred
• Whether patients improve in managing their medications
• Improvements in mobility and movement
• Progress in getting in and out of bed independently
• Improvements in bathing and personal care
• Reduction in shortness of breath
• Prevention of hospital admissions while receiving home health care

These measures are calculated using home health patient assessments and Medicare claims data.

In addition to clinical performance, CMS also publishes a Patient Survey Star Rating, which is based on feedback from the Home Health Consumer Assessment of Healthcare Providers and Systems (HHCAHPS) survey. This survey gathers feedback from patients and families about their experiences with a home health agency, including communication, responsiveness and overall satisfaction.

Most home health agencies receive ratings around 3 to 3.5 stars, which represents the national average. Agencies with ratings above 3.5 stars are performing better than average, making Liberty Home Care’s results a significant achievement.

CMS updates these ratings quarterly, ensuring that the data reflects current performance and patient outcomes.
