Understanding the Hospice Circle of Support
When facing a life-limiting illness, compassionate support can make an immeasurable difference. At Liberty Hospice, patients and families are surrounded by a dedicated circle of support. A full interdisciplinary care team works together to ensure comfort, dignity and peace during one of life’s most meaningful seasons.
What a Hospice Circle of Support Means
Hospice care brings together professionals and volunteers who address the physical, emotional, spiritual and practical needs of both the patient and their loved ones. This holistic approach ensures that families never walk this journey alone.
Meet Your Hospice Team
Hospice Physician
The hospice physician closely monitors the patient’s illness progression, prescribes appropriate medications and coordinates care with all team members. Hospice physicians invite a patient’s preferred physician to stay involved in the care plan if desired.
Hospice Nurse
Hospice nurses assess and manage a patient’s pain, symptoms and disease progression. They also train caregivers who provide hands-on patient care. As skilled listeners, hospice nurses comfort the family while educating on the best goal-oriented care.
Hospice Aide
Hospice aides are certified nursing assistants who provide personal care to the patient, such as bathing or dressing. They are available to ease the burden on family caregivers by participating in activities such as light housekeeping.
Hospice Social Worker
Hospice social workers provide emotional and psychosocial support to the patient and family by assisting with coordination of finances, funeral planning or other tasks. Social workers are always available to lend a friendly and listening ear.
Bereavement Coordinator
The bereavement coordinator addresses both anticipatory grief and loss after death. Families receive bereavement support up to 13 months after a death. Hospice is always available to those who’ve experienced the death of a loved one.
Hospice Volunteer
Hospice volunteers are trained to provide compassionate companionship and support to patients and families. Volunteers are an important part of hospice. Their duties range from visiting patients to providing other levels of support as needed.
Hospice Chaplain or Spiritual Coordinator
Regardless of a person’s beliefs or religious traditions, hospice chaplains are available to address the spiritual issues that often arise during the disease progression. They are there for the patient and family, honoring and supporting their cultural traditions and values.
We’re Here When You Need Us Most
This circle of support is at the heart of Liberty Hospice. Whether managing symptoms, providing comfort or supporting caregivers, the team works together to ensure patients experience dignity and peace while families feel guided and cared for.

Starting the Conversation: Why Discussing Hospice Care Early Matters
Talking about hospice and end-of-life care is never easy, but it’s one of the most important conversations you can have with a loved one. When individuals and families take time to discuss care preferences and document their wishes, it ensures future decisions reflect what truly matters most.
At Liberty Home Care and Hospice, we understand that planning for the end of life can feel overwhelming. Early and thoughtful conversations can lead to more peaceful transitions, better symptom management and care that aligns with your loved one’s values.
Why It’s Important to Talk Early
Many people express a desire to spend their final days at home, surrounded by loved ones. Yet without planning and communication, these wishes can be difficult to fulfill. Discussing hospice options before a crisis occurs allows families to make informed decisions together before serious illness or cognitive decline makes communication more difficult.
Hospice care is not just for the final days of life. In fact, it can begin months earlier, offering pain and symptom relief, emotional and spiritual support and practical help for families. Unfortunately, hospice is often started late because families are uncertain about their loved one’s preferences or unaware that services are available sooner.
Involve Medical Professionals Early
Don’t wait for a healthcare provider to bring up hospice or palliative care. Be proactive. Ask about the prognosis and request clear information about care options, including when a referral to hospice may be appropriate. For those with Medicare, these discussions are covered as part of advance care planning.
Respecting Cultural and Personal Values
Every person’s background, including religious beliefs, cultural traditions and past experiences, can shape how they view end-of-life care. Be patient. Conversations may need to happen more than once, especially if health needs change over time.
Documenting Care Preferences
Once decisions are made, it’s important to record them in a legally recognized format such as a living will, advance directive or healthcare proxy. Liberty Home Care and Hospice assists families with advance care planning through the Five Wishes program. Five Wishes is a living will that covers personal, spiritual, medical and legal wishes all in one document. Regardless of the format, the most critical step is ensuring that caregivers and healthcare professionals know how to access the information when it’s needed.
Each state has different rules about advance care planning, so check with a healthcare provider or legal expert to ensure documents are valid.
Hospice Is About Living with Dignity
Choosing hospice care is not about giving up—it’s about prioritizing comfort, dignity and personal choice. With the right support, patients can spend meaningful time with loved ones and receive care that aligns with their values.
At Liberty Home Care and Hospice, we are here to guide families through every step of this journey. Our interdisciplinary team offers care focused on compassion, respect and quality of life.

What You Need to Know About Five Wishes: Your Guide to Advance Directives

An advance directive is a written, legal document that details your preferences for medical care if you are unable to make those decisions yourself. It serves as a vital guide for your doctor, family members and caregivers, ensuring they know exactly how you wish to be treated.
At Liberty Home Care and Hospice, we primarily use the Five Wishes document to help patients and families plan for the future. Created by the non-profit organization Aging with Dignity and inspired by the work of Mother Teresa, Five Wishes is often called the first "living will with a heart and soul.” It is unique because it addresses your personal, emotional and spiritual needs along with your medical wishes.
The Five Wishes Defined
The Five Wishes document is broken down into five clear sections that allow you to maintain control over your care:
- The Person I Want to Make Care Decisions for Me: You designate a "Health Care Agent" (also known as a proxy or surrogate) to speak for you if you cannot speak for yourself.
- The Kind of Medical Treatment I Want or Don’t Want: This section serves as your living will, where you define what "life-support treatment" means to you and under what circumstances you would want it started or stopped.
- How Comfortable I Want to Be: This section focuses on pain management, methods to improve personal comfort and personal care wishes.
- How I Want People to Treat Me: In this section, you decide personal preferences, such as having music played, being cheered up rather than mourned or having someone hold your hand.
- What I Want My Loved Ones to Know: This section allows you to express love, offer forgiveness and share your wishes regarding funeral arrangements or burial/cremation.
When You Should Complete an Advance Directive
Expressing your wishes now empowers your loved ones to make the best decisions during a crisis and helps avoid family disagreements. The best time to fill out an advance directive is now, while you are mentally competent and before a health crisis occurs. It is recommended that you review your document at least once a year or whenever there is a major change in your health.
Advance Directive Frequently Asked Questions
What is the difference between a Living Will and a Health Care Power of Attorney?
In North Carolina, a Living Will specifically tells others you want to die a natural death if you are terminally ill or in a persistent vegetative state. A Health Care Power of Attorney is the document where you name a specific person (your health care agent) to make medical decisions for you. Five Wishes is a comprehensive document that combines both of these elements.
Is Five Wishes legally valid?
Yes. Five Wishes was created with the help of the American Bar Association’s Commission on Law and Aging. It meets the legal requirements for 46 states, including North Carolina, South Carolina and Virginia.
What are the advance directive signing requirements in North Carolina?
To be legal in North Carolina, your advance directive must be:
- Written and signed by you while you are still able to understand your choices.
- Signed in the presence of two qualified witnesses.
- Notarized by a Notary Public.
Can I change my mind after I sign an advance directive?
Absolutely. You can cancel or change your advance directive at any time. To do so, you should destroy all old copies or write "REVOKED" in large letters across them. You must also inform your doctor and your health care agent of the changes.
Who should I choose as my Health Care Agent?
Choose someone 18 or older who knows you well and is someone you trust to follow your wishes even if they are emotionally difficult. It is important to discuss your wishes with this person before naming them to ensure they agree to respect your choices.
Where should I keep my completed advance directive?
Do not put it in a safe deposit box. Keep the original in a safe but accessible place in your home. Give copies to your health care agent, your doctor and your family members. You should also carry the Five Wishes Wallet Card with you so emergency personnel know who to contact.
When does Fives Wishes take effect?
Five Wishes only takes effect when you are no longer able to communicate. The person you have chosen to be your health care agent will use your Five Wishes document to help convey your specific wishes about your care. If you are still able to communicate, you can still make your own decisions about your care and will not need to use Five Wishes.

How to Start the Conversation About Home Health Care with an Aging Parent

Do you have an aging parent whose health, safety or independence is becoming a concern? If so, you’re not alone. Many families struggle with how to begin the conversation about home health care services. While it may feel uncomfortable at first, talking about senior care options early can lead to better outcomes, greater peace of mind and a smoother transition as your loved one ages.
At Liberty Home Care, we’re here to help guide you through the process with practical tips and compassionate insight. Whether you’re concerned about your parent’s ability to manage daily tasks or just want to ensure they stay safe and comfortable at home, starting the conversation early is key.
Step 1: Prepare for the Conversation
Assess Your Concerns
Before approaching your parent about home health care, it’s helpful to identify specific concerns. Are you worried about falls, missed medications or the ability to handle daily activities like bathing or dressing? These daily activities, often referred to as Activities of Daily Living (ADLs), are a critical factor in determining whether home health care may be necessary.
Include insights from other family members, if possible, to get a full picture of your loved one’s needs. Write down these concerns so you’re prepared to have a clear and thoughtful discussion.
Research Home Health Care Services
Understanding the types of care available is an important step. At Liberty Home Care, home health care services include skilled nursing care, physical, occupational and speech therapy, medical social services and personal care in the comfort of home. Reviewing your options in advance will allow you to answer your parent’s questions and offer well-informed suggestions.
Understand Their Priorities
Most older adults want to age in place and choosing the right in-home care provider can make that possible. Listen to your parent’s preferences and concerns. Independence, safety and comfort are often top priorities. When you lead the conversation with empathy and respect, your loved one is more likely to be receptive.
Step 2: Have the Conversation
Start Early Before a Crisis Occurs
Ideally, these discussions should take place well before a medical emergency. Early conversations give your parent time to process their options and make decisions without pressure. If your parent needs support recovering from illness, injury or surgery, home health care can provide the medical services they need without disrupting their daily routine.
Choose the Right Time and Setting
Whenever possible, have this conversation in person or through a video call, so you can read body language and emotional cues. Find a quiet, comfortable time when you’re both relaxed and free from distractions. This will help keep the conversation focused and positive.
Listen Without Judgment
Let your parent speak openly about their feelings and concerns. Changing living arrangements or accepting help can be emotional. Actively listening to their perspective builds trust and shows that you’re a partner—not just a decision-maker.
Approach with Empathy and Respect
Home health care conversations can be sensitive. Use a warm, calm tone and acknowledge that needing help is a natural part of aging. Reassure your parent that receiving senior care at home doesn’t mean losing independence. It means gaining support that allows them to continue living life on their terms.
Revisit the Conversation as Needed
Don’t expect to resolve everything in one conversation. Instead, view this as an ongoing discussion. Your loved one may need time to consider their choices, consult with their physician and reflect on what matters most.
Respect Their Autonomy
It’s important to remember that, whenever possible, the final decision belongs to your parent. If they are medically able to live safely at home and express a desire to do so, honor that choice. Respecting their independence while offering support can make all the difference in their comfort and well-being.
Final Thoughts
Starting the conversation about home health care services for aging parents can feel overwhelming, but delaying the discussion often leads to rushed decisions in times of crisis. By approaching the topic early, with care and preparation, you empower your parent to make informed choices about their health and lifestyle.

Liberty Home Care Locations Earn Strong CMS Star Ratings for Quality and Patient Experience
Liberty Home Care is proud to share that many of our locations have earned high ratings in the Centers for Medicare & Medicaid Services (CMS) Home Health Star Ratings, reflecting our ongoing commitment to delivering compassionate, high-quality care to patients in the comfort of their homes.
CMS uses a five-star rating system to help patients and families compare home health agencies based on measurable quality outcomes and patient experience. These ratings provide an easy way to understand how agencies perform compared to others across the country.
We are honored that several Liberty Home Care locations received four-star and five-star ratings, demonstrating our dedication to excellent clinical care and positive patient experiences.
What CMS Home Health Star Ratings Measure
The Quality of Patient Care Star Rating evaluates how well home health agencies perform across several important clinical measures. These include both process-of-care measures and patient outcome measures, such as:
- How quickly care is initiated after a patient is referred
- Whether patients improve in managing their medications
- Improvements in mobility and movement
- Progress in getting in and out of bed independently
- Improvements in bathing and personal care
- Reduction in shortness of breath
- Prevention of hospital admissions while receiving home health care
These measures are calculated using home health patient assessments and Medicare claims data.
In addition to clinical performance, CMS also publishes a Patient Survey Star Rating, which is based on feedback from the Home Health Consumer Assessment of Healthcare Providers and Systems (HHCAHPS) survey. This survey gathers feedback from patients and families about their experiences with a home health agency, including communication, responsiveness and overall satisfaction.
Most home health agencies receive ratings around 3 to 3.5 stars, which represents the national average. Agencies with ratings above 3.5 stars are performing better than average, making Liberty Home Care’s results a significant achievement.
CMS updates these ratings quarterly, ensuring that the data reflects current performance and patient outcomes.

National Caregivers Day: Celebrating the Compassion of Those Who Care for Others
National Caregivers Day, observed annually on the third Friday of February, is a time to honor the remarkable individuals who dedicate their time, energy and hearts to caring for others. This year, Liberty Home Care and Hospice recognizes the millions of Americans who provide essential—and often unseen—support to loved ones facing health challenges.
Caregivers play a vital role in maintaining the physical, emotional and social well-being of those they assist. Their kindness and dedication strengthen families, neighborhoods and entire communities.
The Essential Role of Caregivers
Caregivers manage a wide range of responsibilities, including:
- Medical assistance and medication management
- Personal grooming and hygiene
- Meal preparation and household upkeep
- Transportation and scheduling
- Social engagement and companionship
- Coordinating care with doctors, hospitals and insurance teams
While these tasks are demanding on their own, many caregivers juggle them alongside full‑time careers and family commitments.
Caregiving Continues to Rise Across the Nation
Caregiving in the United States has expanded significantly over the past decade, with new data highlighting just how essential—and challenging—this role has become.
According to the Caregiving in the U.S. 2025 report from the National Alliance for Caregiving (NAC) and AARP, an estimated 63 million Americans—about 1 in 4 adults—are unpaid family caregivers, a 45 percent increase since 2015.
The 2025 survey highlights the growth of millennial and Gen Z caregivers, noting that 29 percent of caregivers fall into the “sandwich generation” caring for both children and adults. Among caregivers under 50, that share rises to 47 percent. According to the survey, 64 percent of caregivers report emotional stress.
These updated statistics show that caregivers are doing more than ever and facing growing challenges that deserve recognition and support.
How to Show Appreciation on National Caregivers Day
If someone in your life is a caregiver, here are meaningful ways to express gratitude:
- Share a heartfelt message: A personal letter or card can remind caregivers that their work is seen and appreciated.
- Gift something comforting: Small items like books, baked treats, candles or soothing lotions can offer moments of peace.
- Offer a much‑needed break: Your time can be a gift. Providing a few hours of respite can give caregivers the chance to rest and recharge.
- Encourage them to access support resources: Caregivers often don’t ask for help. Sharing information about respite care or support programs can make a world of difference.
Respite Care Support Through Liberty Hospice
For our Liberty Hospice families, we provide respite care that allows caregivers to rest, take a vacation or simply regain balance.
Respite care provides up to five consecutive days of inpatient care at a Medicare-certified nursing facility or hospital, where the patient’s hospice interdisciplinary team will continue to visit them and provide care as needed. We also provide counseling and support services to families.
A Message of Gratitude
At Liberty Home Care and Hospice, we extend our deepest appreciation to every caregiver who supports their loved ones, neighbors and friends. Your compassion is powerful. Your dedication is inspiring. Your kindness deserves to be celebrated—not only on National Caregivers Day, but every single day.
