Liberty Hospice Locations Achieve Level 3 We Honor Veterans Status
Liberty Hospice is proud to announce that 13 office locations have recently achieved Level 3 Partner status with We Honor Veterans, a national initiative led by the National Hospice and Palliative Care Organization (NHPCO) in collaboration with the Department of Veterans Affairs (VA).
Our Burlington, Dunn, Durham, Fayetteville, Jacksonville, Roanoke Rapids, Sanford, Siler City, Smithfield, Supply, Swanquarter, Wendell, and Wilmington offices now hold Level 3 Partner status, further demonstrating Liberty Hospice’s dedication to ensuring Veterans and their families receive the highest quality hospice care across North Carolina. This accomplishment reflects our ongoing commitment to recognizing, honoring and providing compassionate, specialized care for the Veterans who have bravely served our country.
What Level 3 We Honor Veterans Status Means
Achieving Level 3 Partner status is a significant milestone in the We Honor Veterans program. It requires hospices to go beyond basic recognition of Veterans by actively developing programs, policies and partnerships that ensure Veterans receive care tailored to their unique experiences.
To earn this certification, Liberty Hospice teams have completed a variety of Veteran-focused education, outreach and partnership activities, including:
- Conducting Veteran-specific training sessions for hospice staff and volunteers
- Integrating military history checklists into patient intake and care planning
- Developing and implementing a Veteran-to-Veteran Volunteer Program to connect Veteran patients with fellow Veterans for meaningful support
- Hosting Veteran-specific outreach presentations within the community and healthcare settings
- Building strong partnerships with VA Medical Centers and Veteran Service Organizations to ensure Veterans and their families have access to essential benefits and resources
